Three assessments, three depths. Each one is complete on its own. Start with a deep dive or a one-off test.
The long game of your health.
A physician-led longevity practice for people who want early risk clarity, deep evaluation, and long-term follow-through.
Choose your assessmentA simple model for long-term health
Most medicine reacts after disease appears. We work earlier, with structured evaluation, planning, and follow-through across the year.

Aeternitas is a direct-pay practice with a deliberately limited membership panel. Care begins at our flagship clinic in Charlotte, NC, with virtual follow-up available where licensure allows.
Choose your assessmentOne coordinated day
to measure what matters.
The Comprehensive and Advanced Longevity Assessments are built around one day at the clinic. About four and three-quarter hours. Every specialist sees you, and Dr. Helton reads everything. It is where the Foundation and Momentum memberships begin. The Core Longevity Assessment is a shorter physician-and-labs visit and does not include this day.
*This procedure is provided off-siteHeart imaging
Comprehensive includes a coronary artery calcium score when clinically appropriate. Advanced includes coronary CT angiography. If Dr. Helton decides CT angiography is not appropriate for you, its value is refunded.

Blood and biomarker analysis
One blood draw at the start of the day covers heart risk, metabolic health, inflammation, and hormones. Every result is read against what it means for your long-term health.

Performance & movement
A VO₂ max test with your own training zones, a movement screen, and grip strength, run by a clinical exercise physiologist. You learn how much capacity you have and where to build it.

Brain & sensory health
Standardized neurocognitive testing and hearing assessment to establish a baseline and identify early changes in brain and nervous system function.

Body composition
We measure body composition with DEXA (coming soon)

Results visit and written plan
Results visit with Dr. Helton and the team. You always leave with a clear written plan.
A simple framework.
Applied rigorously.
Every recommendation is grounded in four essentials: what’s likely to happen, what’s true today, what can change it, and what you’ll actually follow.
Risk outlook
We map your biggest long-term risks using your history, family history, and testing.
Health baseline
A deep baseline across cardiovascular, metabolic, brain, and recovery systems
Interventions
Exercise, nutrition, sleep, medication, and behavior, applied based on your data.
Life alignment
We build around your priorities, schedule, stress, and motivation so it holds.
When all four stay in view, progress becomes repeatable.
We focus on the interventions
that change outcomes.
Your plan is built around five strategies that meaningfully influence long-term health. Each is guided by a clinical specialist and coordinated by your physician, so changes are precise, intentional, and sustainable.
Pharmacotherapy
Targeted use of medications and medical therapies directed by your physician when evidence, risk, and preferences align.
Nutrition
Nutrition strategy overseen by a registered dietitian, shaped by your biomarkers, risk profile, and real-world constraints.
Sleep
Sleep interventions informed by clinical assessment and, when appropriate, objective testing to support recovery and long-term health.
Mental health & behavior
Support from a licensed therapist focused on stress load, emotional health, and behavior patterns so plans hold up in real life.
Exercise
Strength, conditioning, and movement guided by an exercise physiologist to preserve muscle, protect joints, and improve cardiovascular and metabolic capacity.
Built for people who plan the
second half of life on purpose.
A disciplined, high-touch model built for long-term execution.
Longevity minded
We plan in years, not weeks.
Evidence over trends
We adapt as the science evolves.
Precision science
Testing guides decisions. Nothing is for show.
Clarity you can follow
You’ll understand the why, not just the what.
Structure that holds
A steady rhythm that makes consistency realistic.
Calm by design
A limited panel. A refined space. Unhurried time.
Three ways to work with us.
Buy a test, a 12-week program, or a year of physician-led care. Every path starts from your own results, and none of them requires the others.
Measure
Testing & assessments
Focused testing and one-time assessments. You leave with your numbers, an expert’s read of them, and a written next step. No membership required.

Dr. Corey Helton, MD
Physician
Dr. Corey Helton is a board-certified family physician and the founder of Aeternitas. After years as a hospitalist caring for patients at the late stages of preventable disease, he built Aeternitas to move medicine upstream: measuring risk earlier, interpreting it clearly, and turning it into a structured, long-term plan. His work is grounded in a simple belief, that the most compassionate medicine helps people preserve their strength, clarity, and independence long before a crisis ever arrives.

Licensed Marriage & Family Therapist · Emotional Health
Cait Helton is a licensed marriage and family therapist with advanced training in trauma and EMDR, and the architect of the emotional health program at Aeternitas. Frequently Asked Questions
I already have a doctor. Why would I need this?
You can keep your doctor. A test, assessment, or 12-week program does not replace anyone. You leave with your numbers, a physician-reviewed report, and a written next step, and the report is yours to share. Membership is different. There, Dr. Helton becomes your primary care physician, and you do not need to keep another one.
Is this a wellness clinic or functional medicine?
No. Aeternitas is a physician-led, longevity-focused medical practice. It is not alternative medicine. Every test we run has a reason, and every reason is backed by science. We do not order a long list of labs hoping something turns up, and we do not chase a single "root cause" when medicine cannot name one. We use the best evidence available, and we follow it further than a standard checkup does. When the guidelines run out and you are the person in the room, a physician uses judgment. That is the difference. Dr. Helton reviews every report before you see it. We name what we measure, and we do not promise outcomes.
Do I have to join a membership to book something?
No. You can buy any test, assessment, or program on its own. None of them requires a membership. If you complete an assessment and decide to become a member later, you can roll the cost of your assessment into the membership.
How does my assessment count toward a membership?
Enroll within 90 days of getting your results and plan, and what you paid for the assessment counts toward your membership. The exact figures are confirmed at enrollment. Dr. Helton confirms every enrollment through your assessment.
