Primary care that plans the decades ahead

Dr. Helton is your primary care physician. He holds your whole record, manages your medications, sees you when you are sick, and builds your plan from your own numbers. A year of care, in three tiers.

It begins with an assessment, so your care starts from your own numbers.

Why Aeternitas

A short visit and a normal lab report is not a plan.

Maybe your care is spread across a cardiologist, an endocrinologist, and a clinic that sees you for a few minutes a year. Maybe you manage a condition well enough, and no one has looked at how the pieces fit. Maybe you feel fine, and your last physical ended with “everything looks good” and no plan.

Different starting points. The same gap: no one owns the whole picture, and no one is planning for what comes next.

Aeternitas exists to close that gap.

Dr. Corey Helton

One physician who holds the whole picture

Dr. Helton is your primary care physician. He manages your medications on your own labs, sees you when you are sick, and coordinates every specialist. No set visit count. No per-visit fee. And you have his personal cell.

When a question comes up, you reach the physician who already knows your labs, your history, and your goals — not a portal, not a triage line, not whoever is on call.

  • Your exercise plan doesn’t come from a doctor.

It comes from an exercise physiologist.

  • Your nutrition plan comes from a dietitian.

Built from your own labs and your own habits, not a template.

  • Our therapist helps you think about longevity and alignment.

The part of the plan that decides whether the rest of it happens.

  • One physician reads all of it.

    And owns what happens next.

  • You leave holding a written plan.

Your priorities in order, and who works on each. Not forty findings.

Your first 60 days

It begins with an assessment. Sixty days later you have your numbers, a plan, and a decision to make.

  1. / 01

    Your Intake

Complete a detailed intake so we can really get to know you.

  2. / 02

    Your clinic day.

Several hours with the team. All about you.

  3. / 03

    The at-home phase.

A glucose monitor, a home sleep study, and genetics, over four to six weeks.

  4. / 04

Your results visit with Dr. Helton.

He walks you through what your numbers mean and what to do first.

  5. / 05

Your written plan, and your decision.

If membership fits, it starts from there.

  6. When your results and plan are delivered, you decide. Enroll within 90 days and what you paid for the assessment counts toward your membership.

What happens on the clinic day

Timelines are targets. They depend on scheduling, intake completion, outside records, and result turnaround.

Straight answers before you commit

What each tier includes, where it stops, and the rules every tier shares. Answered here, not on an invoice.

Anchor

What you get: Dr. Helton as your primary care physician all year, from a focused baseline. You do not give up a day for testing.

Where it stops: no specialist team, and no genetics, glucose, or sleep testing. Those can be added as needed at member pricing.

Foundation

What you get: the full Evaluation Day with the whole team, a year of primary care, and one 12-week program chosen from your plan, run by the clinician who assessed you.

Where it stops: one program is inside the price. A second is a separate purchase at member pricing.

Momentum

What you get: a deep longevity assessment to start, then exercise, nutrition, and mental health managed all year, with the whole team checking in every quarter. You never need to buy the aerobic or nutrition programs.

Where it stops: one EMDR intensive is included when clinically indicated. Therapy beyond it is member-priced.

In every tier

  • No per-visit fees. Sick visits and follow-ups happen as often as your health needs them.
  • You pay what we pay for deeper screens. Dr. Helton recommends them case by case, billed at exact vendor cost, no markup.
  • Member pricing on everything else: 35% off tests, 15% off assessments and programs. Move up a tier through month eight and pay only the difference.
  • Not included: medication costs, and after-hours coverage.

You risk an assessment, not a year.

The year is your decision, made with your own results in hand. Four rules make the first step safe.

  1. / 01

The assessment stands on its own.

Buy one and you owe nothing further. Your results and your plan are yours to keep.

  2. / 02

    What you paid counts.

Enroll within 90 days of your plan, and what you paid for the assessment counts toward your year.

  3. / 03

    You can change your mind.

Withdraw before your results visit and we refund the membership portion you paid, less any care already delivered.

  4. / 04

    Fit comes first.

Dr. Helton confirms every enrollment through your assessment. If it is not right for you, you are not enrolled, and you choose a transfer or a full refund.

Frequently asked questions

Yes. In every tier, Dr. Helton is your primary care physician. You do not need to keep another one. Our team is available Monday through Friday, 8 a.m. to 5 p.m. We do not provide after-hours, on-call, or emergency coverage.

As often as your health needs it. There is no set visit count and no per-visit fee. Sick visits and follow-ups happen when they are clinically needed, and he manages your medications from your own labs. Each tier adds its own planned rhythm on top: Foundation adds one 12-week program, and Momentum adds quarterly check-ins with the whole team.

You can, and many people do. Membership adds the part you cannot buy one at a time: a year of primary care with a physician who has read all of it. It also brings member pricing on every test, assessment, and program. Foundation includes one program. Momentum includes exercise and nutrition support all year. And what you paid for your assessment counts toward it.

No. Membership is a 12-month agreement, paid quarterly or in full. The monthly number on this page is for comparison only. If you enroll within 90 days of getting your results and plan, what you paid for your assessment counts toward your membership. The exact figures are confirmed at enrollment.

Buying an assessment and joining a membership are two separate decisions. You buy the assessment first. It is complete on its own, and you owe nothing more. After your results visit, if a membership fits, we offer it to you, and what you paid for the assessment counts toward it. You can say no and keep your results and your plan. If you say yes, you sign a 12-month agreement, and that commitment stands for the year. One exception: if you choose to enroll early, before your results visit, and then change your mind, we refund the membership portion you paid, less any care already delivered.
